Credit Card Verification Form

As per the Credit Card Transaction Processing Agreement, this form may be required to be submitted for each Credit Card used in your account prior to the processing of any payouts.

Fax Transmissions

• The international direct dialing (IDD) prefix associated with your country must be included before our fax number.
• To find the IDD prefix associated with your country, please click here.
• When transmitting your fax, replace the '+' in our fax number with your country's direct dialing prefix.

Tips to ensure your documentation is received properly

• Set both your photocopier and fax machine on the lightest possible setting to ensure legibility.
• Ensure all supporting documentation, as outlined at the top of each verification form, is included with your fax.
• When submitting documentation for multiple Credit Cards, a separate form is required for each card.
• Fill out, sign and date the form where indicated.
